Faculty Practice Sites & Initiatives


1. Michigan State University Family Health Center

Description: an integrated, collaborative primary care center that combines the practices of physicians in the College of Human Medicine Department of Family Medicine and nurse practitioner faculty in the College of Nursing. The practice is located on campus in A-142 Clinical Center, Service Drive, East Lansing, MI.

2. Ingham County Jail-Primary Care

Description: a collaborative agreement with the Ingham County Health Department to provide contracted nurse practitioners to direct primary care services for inmates of the Ingham County Jail. The jail is located on N. Cedar Street in Mason, MI.

3. Mid Michigan Physicians PC-Obstetrics & Gynecology

Description: a collaborative agreement with Mid Michigan Physicians- Obstetrics & Gynecology practice to provide a contracted nurse midwife for direct patient care in the office setting at 1540 Lake Lansing Road, Suite 205, Lansing, MI.  This contracted service includes the full scope of nurse midwifery practice, excluding newborn delivery. This practice is in collaboration with Frank Takyi, M.D., Obstetrics & Gynecology physician.

4. Group Visits-Diabetes

Description: a collaborative project with Michigan State University HealthTeam primary care providers in partnership with Blue Cross Blue Shield of Michigan and their performance-based rewards program to improve outcomes of individuals with diabetes. This project is focused on client self-management, goal setting, and lifestyle modification to achieve optimal health. Each monthly session includes the services of a nurse practitioner provider and registered nurse facilitator.
